We have been hard at work getting our clinic set up this week! We have a new patient registration pack.
We plan to be open for Primary and Convenient Care starting at the beginning to the middle of June.
We will be accepting most major insurances including Medicare, Medicaid, and L&I. We will also be accepting cash payments on a sliding scale.
Registering as a new patient
If you are interested in registering as a new patient email us at registeration@rivervalleyhealth.org or call us at 360-506-8352 and leave a message with your name, date of birth, address, and phone number. We will mail you a packet of new patient paperwork.
